The Intolerance of Uncertainty Scale – Short Form (IUS-12) was revised by Carleton, Norton, and Asmundson, 2007.
As the authors published the full questionnaire in their article, which is publicly available online, I am attaching you the original article and a Word file containing the IUS-12 version we used in our study.
Good luck with your study!
Best,
Einav
