The following authorities approved the study: The Regional Committee for Medical and Health Research Ethics (REK) of Health Region South-East, Norway (2015/96). The Institutional Review Board of Oslo university hospital (2015/1868). The National Medicines Agency (15/04932-7). The study was registered in Oslo University Hospital Research Registry (ePhorte 2015/1868), and conducted according to the ethical standards of the Helsinki Declaration (1975 and as revised in 1983). Study participants were included after written and oral informed consent. The study follows a prospective and observational design.
