Approval for the present study was obtained from the South Birmingham Local Research Ethics Committee. All participants gave written informed consent for their respective biopsies to be utilized in this study. Approval for further anaylsis was granted by the Liverpool Tissue Bank Ethics Committee (application no. 12-09). All participants gave written informed consent for their respective biopsies to be used in biomedical analysis.
