Demographic and clinical data were collected by research nurses using a case report form completed in person and corroborated against the patient’s medical notes. Follow-up clinical data were collected in the same manner at 3-6 monthly intervals. eGFR was calculated using the 2009 Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation.10 For patients who lacked urinary albumin-creatinine ratio (uACR) data, uACR values were estimated from urinary protein-creatinine ratios using the equation described by Weaver et al.11
