The 5 level EQ-5D was used in this study. The EQ-5D questionnaire consists of two parts. The descriptive system assesses five different aspects of health: mobility, self-care, usual activities, pain/discomfort, and depression/anxiety on a 5-point Likert scale.9,15 A key feature of the EQ-5D is the availability of “value sets” to weight the health states reported by patients into utility indexes (EQ-5Dindex). We used the UK value set in this study because a Danish value set is not yet available. These values range from -0.285 to 1.0, a value of 1.0 corresponds to full health, 0 corresponds to death and negative values correspond to health status considered to be worse than death.10 In addition, the EQ-5D contains a visual analogue scale (EQvas) that records self-rated health in a single score ranging from 0 to 100.8,9
