This questionnaire was carefully constructed and based on data collected from the Fourth National Oral Health Survey [10], incorporating a broad range of questions designed to provide a comprehensive and relevant analysis of respondents’ oral health status. The questionnaire encompassed a variety of facets, including the general information of the participants, their oral health routines, indicators of poor oral hygiene, and the degree of oral health awareness within their families. By implementing such a thorough questionnaire, our primary aim was to guarantee a comprehensive understanding of the examinee’s oral health condition, thereby facilitating a comprehensive approach to their dental examination.

A comprehensive review process was undertaken by an experienced auditor, who meticulously scrutinized each answer to detect and rectify any possible errors or omissions. This meticulous review aimed to uphold the utmost reliability and accuracy of the gathered data. For the sake of maintaining the highest standards of data integrity, any responses that were determined to be incomplete or unsatisfactory were discarded. This ensured the results were reliable and valid, providing useful insights to organizations and individuals.
