Tissue samples were fixed in cacodylate-buffered 2.5% (w/v) glutaraldehyde, postfixed in 2.0% osmium tetroxide, then embedded in epoxy resin. Ultrathin sections were poststained with uranyl acetate and lead citrate. Imaging was performed using a JEOL 1010 electron microscope fitted with a Hamamatsu digital camera and AMT Advantage imaging software. Micrographs were assessed by 2 investigators blinded to clinical diagnosis.
